Entries on the first portion card include name, age, nativity, marital status, residence, and the address of nearest relative. It then asks for soldier rank, company, regiment, and the date admitted. The next section has a large box to fill in the diagnosis, then asks for the occasion wounded, the date, and the nature of missile or weapon. On the back of the card there are two large spaces to write in the treatment used and the results.
